TEHRAN (Iran News) – A pattern is emerging among COVID-19 patients arriving at hospitals in New York: Beyond fever, cough, and shortness of breath, some are deeply disoriented to the point of not knowing where they are or what year it is.

At times this is linked to low oxygen levels in their blood, but in certain patients, the confusion appears disproportionate to how their lungs are faring, AFP reported.

Jennifer Frontera, a neurologist at NYU Langone Brooklyn hospital seeing these patients, said the findings were raising concerns about the impact of the coronavirus on the brain and nervous system.

By now, most people are familiar with the respiratory hallmarks of the COVID-19 disease that has infected more than 2.2 million people around the world.

But more unusual signs are surfacing in news reports from the frontlines.

A study published in the Journal of the American Medical Association last week found 36.4 percent of 214 Chinese patients had neurological symptoms ranging from loss of smell and nerve pain to seizures and strokes.

A paper in the New England Journal of Medicine this week examining 58 patients in Strasbourg, France found that more than half were confused or agitated, with brain imaging suggesting inflammation.

“You’ve been hearing that this is a breathing problem, but it also affects what we most care about, the brain,” S. Andrew Josephson, chair of the neurology department at the University of California, San Francisco said.

“If you become confused if you’re having problems thinking, those are reasons to seek medical attention,” he added.

“The old mantra of ‘Don’t come in unless you’re short of breath’ probably doesn’t apply anymore.”

Viruses and brain

It isn’t completely surprising to scientists that SARS-CoV-2 might impact the brain and nervous system since this has been documented in other viruses, including HIV, which can cause cognitive decline if untreated.

Viruses affect the brain in one of two main ways, explained Michel Toledano, a neurologist at Mayo Clinic in Minnesota.

One is by triggering an abnormal immune response known as a cytokine storm that causes inflammation of the brain — called autoimmune encephalitis.

The second is the direct infection of the brain, called viral encephalitis.

How might this happen?

The brain is protected by something called the blood-brain-barrier, which blocks foreign substances but could be breached if compromised.

However, since the loss of smell is a common symptom of the coronavirus, some have hypothesized the nose might be the pathway to the brain.

This remains unproven — and the theory is somewhat undermined by the fact that many patients experiencing anosmia don’t go on to have severe neurological symptoms.

In the case of the novel coronavirus, doctors believe based on the current evidence the neurological impacts are more likely the result of overactive immune response rather than brain invasion.

To prove the latter even happens, the virus must be detected in cerebrospinal fluid.

This has been documented once, in a 24-year-old Japanese man whose case was published in the International Journal of Infectious Disease.

The man developed confusion and seizures, and imaging showed his brain was inflamed. But since this is the only known case so far, and the virus test hasn’t yet been validated for spinal fluid, scientists remain cautious.

More research needed

All of this emphasizes the need for more research.

Frontera, who is also a professor at NYU School of Medicine, is part of an international collaborative research project to standardize data collection.

Her team is documenting striking cases including seizures in COVID-19 patients with no prior history of the episodes, and “unique” new patterns of tiny brain hemorrhages.

One startling finding concerns the case of a man in his fifties whose white matter — the parts of the brain that connect brain cells to each other — was so severely damaged it “would basically render him in a state of profound brain damage,” she said.

The doctors are stumped and want to tap his spinal fluid for a sample.

Brain imaging and spinal taps are difficult to perform on patients on ventilators, and since most die, the full extent of neurologic injury isn’t yet known.

But neurologists are being called out for the minority of patients who survive being on a ventilator.

“We’re seeing a lot of consults of patients presenting in confusional states,” Rohan Arora, a neurologist at the Long Island Jewish Forest Hills hospital told AFP, saying that describes more than 40 percent of recovered virus patients.

It’s not yet known whether the impairment is long term, and being in the ICU itself can be a disorienting experience as a result of factors including strong medications.

But returning to normal appears to be taking longer than for people who suffer heart failure or stroke, added Arora.

TEHRAN (Iran News) – South Korea is pushing for a resumption of humanitarian trade transactions with Iran amid the coronavirus outbreak, with the shipments likely to begin next month, a South Korean Foreign Ministry official said on Friday.

The South Korean official, speaking on condition of anonymity, said that South Korea has won US approval for the resumption of humanitarian exports to Iran under a special license program, according to Yonhap News Agency.

On Monday, Seoul received the green light for the exports based on the US government’s General License No. 8 – a mechanism to authorize certain humanitarian transactions with Iran even if they involve Iran’s central bank subject to US sanctions.

“On April 6, the humanitarian export process based on the General License No. 8 got underway,” the official told reporters.

“Our companies and banks should prepare documents needed to carry out enhanced due diligence, and we think that the shipments may begin about a month later,” he added.

Apart from the licensed program, South Korea is also pushing for the Korean Humanitarian Trade Arrangement (KHTA), which uses an Iranian bank free from US sanctions – such as the Middle East Bank – to facilitate humanitarian transactions with the Islamic republic.

South Korea is also exploring ways to use the Swiss Humanitarian Trade Arrangement, a payment method designed to facilitate Swiss companies’ sales of food and medicine to Iran, to carry out its transactions with Iran.

“South Korea is pushing for all three methods,” the official said, noting that the General License No. 8 platform appears to be the fastest way to resume humanitarian exports to Iran, which has faced shortages of medicine and tools to fight the new coronavirus due to US sanctions.

Though humanitarian exports are not subject to sanctions, South Korean firms had difficulty resuming their sales of medicines and other products to Iran due to fears that they could be affected by the American sanctions.

Remarks by the South Korean Foreign Ministry official came days after the country’s embassy in Tehran in a Twitter message said that Seoul plans to send a medical consignment worth $2 million to Iran to help the Islamic Republic contain the coronavirus outbreak.

“Wishing for the eradication of the coronavirus disease and getting better days, the Republic of Korea intends to deliver a medical consignment worth $ 2 million from the Korean people to the Islamic Republic”, the embassy said in its message on Tuesday.

It added that medical items worth 200,000 dollars, including PCR sets and disinfectant pumps have already been delivered to Iran, the worst-hit country by the disease in the Middle East.

South Korea was one the hardest-hit countries outside China at the beginning of the coronavirus outbreak originated from the Chinese city of Wuhan in the Hubei Province. It has managed to drastically reduce the number of new coronavirus infections.

Several countries have so far shipped humanitarian aid to Iran which its economy has been damaged by the US sanctions reimposed in 2018 following Washington’s unilateral withdrawal from the 2015 Iran nuclear agreement.

Iran’s Foreign Ministry spokesman Seyyed Abbas Mousavi said on Monday more than 30 countries, as well as a number of international organizations, have provided Iran with assistance in its battle against the coronavirus outbreak.

The pandemic, which has afflicted more than 200 countries worldwide so far, resulted in displays of solidarity by the international community and among Iranians, the spokesman said.

At the height of the virus outbreak in China, Iran sent several aid shipments to the country which has sent several humanitarian aid consignments to Iran.

Chinese government sent its ninth aid shipment to Iran on Monday. It has so far sent 28 planeloads of humanitarian aid to Iran from the cities of Beijing, Shanghai, and Guangzhou.

Making the remarks in a parliament session, Jahangiri said that “with the issued permit by Ayatollah Khamenei, allowing withdrawal of one billion euros from National Development Fund Of Iran (NDFI) to combat the coronavirus in the country, a large volume of the economic difficulties will be removed.”

“Under the COVID-19 outbreak, the government is to pay 23 million to Iranians for their losses. These families are the ones who have been included in the cash subsidies scheme of the government, about ten million rials (about $60.6),” he said. “The families will return the subsidy in two years.”

As he explained, the government also plans to pay some subsidies to private sector economic firms who have been hit by the outbreak.

Jahangiri had earlier vowed that the government will provide all its amenities and facilities required for combating coronavirus in the country.

In his remarks on the same day, Iranian President Hassan Rouhani said that the Iranian nation has proved their endurance in the past difficulties and will definitely overcome the current challenges with reliance on their own capabilities and with the help of the friendly countries.

Over 62,000 people have so far been infected with the new coronavirus in Iran, and the death toll has neared to 4,000.

“Due to an impressive scaling up of many of the control measures, we have seen a flattening off of the number of cases in Iran and in fact, some suggestion, in recent days or perhaps a decline in the number of new cases,” Richard Brennan, Acting Regional Emergency Director for WHO EMRO, said at a press conference Tuesday, according to Daily News.

On March 11, Richard Brennan hailed the preventive measures taken by the Islamic Republic of Iran in universities, academic centers and metropolis Tehran in the fight against outbreak and containing coronavirus, also known as COVID-19.

Iran’s Health Ministry Spokesman Kianoush Jahanpour said on Tuesday that the total number of confirmed coronavirus cases in Iran has reached 62,589, with a total death toll of 3,827.

TEHRAN (Iran News) – China has reported zero cases of infections with the new coronavirus Covid-19 outside Hubei Province, the epidemic’s epicenter, for the first time since the outbreak started in December 2019 and later reached other world continents, where the disease continues to infect more people and take more lives.

The novel coronavirus, which first emerged in the city of Wuhan in Hubei, has killed more than 3,800 people and infected over 108,000 in at least 95 countries — the majority in mainland China.

The country’s National Health Commission on Sunday recorded 40 new cases, down from 44 a day earlier, the lowest since Beijing first began to release data on January 20.

Of the new infections, 36 were in Wuhan and the remaining four had been imported from Iran, one of the worst-hit countries.

On Sunday, China also reported 22 new deaths — which were all, except one, in Hubei, bringing the toll to 3,119.

More than 80,700 people have in total been infected in mainland China.

China closed several makeshift hospitals for coronavirus patients and some schools reopened on Monday as normality slowly returns to the country after weeks of battling the epidemic.

Last week, a top government official said that a lockdown imposed on some 56 million people in Hubei since late January could soon be lifted.

On the contrary, the global fight against the outbreak has been stepped up since the weekend as more people are contracting Covid-19.

Italy

Italy, which has stood alone as a hot zone in Europe, placed a quarter of its population on lockdown on Sunday.

Health officials reported a huge jump in deaths from the disease on Sunday, with Prime Minister Giuseppe Conte saying that the country is “facing an emergency, a national emergency.”

Cases of coronavirus almost tripled from about 2,500 infections on Wednesday to more than 7,375 on Sunday across the country.

The death toll also rose from 233 to 366, a rise of more than 50 percent in 24 hours.

As part of effort to limit the transmission of COVID-19, Pope Francis recited the Angelus prayer via Livestream on screens on Saint Peter’’s Square in Vatican City on Sunday.

The traditional Angelus normally attracts large numbers of people to Saint Peter’s Square.

“To recite the Angelus this way is a safety measurement to avoid overcrowding and the possible spread of the virus,” he said.

Meanwhile, the number of confirmed cases has also been on the rise in all of Italy’s neighbors.

French health authorities reported three new deaths on Sunday, bringing the toll to 19. The number of reported infections also rose to 1,126 cases.

The country, which is currently at a coronavirus alert level 2, has announced a ban on gatherings of more than 1,000 people.

In Germany, fears of the virus’ spread have prompted the government to call for the cancelation of large events.

As cases reached close to 850 in the country, Health Minister Jens Spahn said Sunday that gatherings of more than 1,000 participants should be scrapped.

In another development, Berlin and Paris withdrew their representatives from North Korea and closed their missions in Pyongyang on Monday, according to the British ambassador there.

North Korea has not confirmed any cases of the coronavirus, but it has made foreigners from any country that has reported a case to spend 30 days in quarantine. It has also reinforced border checks.

Turning point nears in virus fight: South Korea

Across the North’s border, South Korea has reported 69 new cases.

The Korea Centers for Disease Control and Prevention (KCDC) said Monday the new cases brought the country’s total infections to 7,382, while the death toll rose by one to 51.

Authorities expressed hope on Monday that the country was nearing a “turning point” in the crisis as the rate of new infections fell to its lowest in 10 days on Sunday.

Vice Health Minister Kim Gang-lip, however, warned that it was premature to say the crisis was over.

“There are still many patients arising from Daegu and nearby regions … and sporadic infections continue to emerge elsewhere, though they’re not spreading as fast,” Kim said.

The South has imposed mutual travel restrictions with Japan in a move that has sparked a diplomatic and economic feud between the two.

Seoul suspended visas and visa waivers for Japan on Friday after Tokyo announced travel restrictions from South Korea.

According to data gathered by Japanese local governments, the viral infection has increased to 483 as of Monday, after three more cases were reported earlier in the day.

A total of 14 people have also died from the illness in the country.

Middle East

In the Middle East, Saudi Arabia has become the latest country to announce new cases of the disease.

The Health Ministry said on Monday there were four new cases of coronavirus, taking the total infections to 11 in the kingdom.

Kuwait said it had discovered one new case on Sunday, bringing the number of those infected in the country to 62, according to the Kuwaiti Health Ministry.

Meanwhile, the number of cases in Lebanon and Palestine stands at 22 each.

The total number of confirmed cases in Iraq climbed to 54 on Sunday, according to health officials, who also confirmed two further deaths due to the coronavirus. This took the total number of deaths in Iraq to six.

Iran has been the worst-hit country in the Mideast, with a total of 6,566 infected cases and a total of 194 deaths on Sunday. At least 2,134 people have also been released from hospitals after recovering from the illness.

Egypt reports first death

Egypt’s Health Ministry reported the country’s first death from the coronavirus on Sunday.

It said that a 60-year-old German tourist, who had arrived in Egypt seven days ago and was taken to hospital in the Red Sea resort of Hurghada, passed away on Sunday.

The ministry said in a statement that officials registered seven new cases – four foreigners and three Egyptians on Sunday.

This brought the total number of confirmed cases to 55 in Egypt.

TEHRAN (Iran News) – Persepolis Irish forward Anthony Stokes says that he has not returned to Iran because of the coronavirus outbreak.

The 31-year-old forward was supposed to join Persepolis one day after the Iranian team met Emirati side Sharjah in the AFC Champions League on February 18 but went AWOL.

Now, Stokes has broken his silence after about one month.

“Let me be clear. I have not left Persepolis. I was in Dubai and all flights to and from Iran were stopped, the borders were also closed. I could not get into the country because of the coronavirus outbreak. Now in Iran, all my clubs’ games and social events have been ceased while they try to contain the virus spreading. I am just waiting to make sure it’s safe to return. Simple as that. I have no intentions of traveling to a country until I know it is 100% safe, anyone in my position would do exactly the same thing,” he wrote in a post published on his Instagram.
